Home First (West)
Home First West is an assessment and reablement service designed to assist frail or physically challenged individuals to regain skills that maximise independence and improves their wellbeing. Assessments are usually for a maximum of 2 weeks and reablement can be from a few days and up to a maximum of 6 weeks, some of which can be free of charge depending on the individual circumstances. Referrals can be made through Customer First on 08088004005 or through your G.P, Social Worker or the NHS. This service is a Suffolk County Council driven Domiciliary Resource and therefore cannot accept private paying Customers. Home First West has a Care Quality Commission Rating of OUTSTANDING from their last inspection in July 2019.
