Acute Liaison Nursing service for Adults with Learning Disabilities (Norfolk and Norwich University Hospitals NHS Foundation Trust)

The Liaison nurses work with people over the age of 16 who have learning disabilities and/or autism spectrum conditions, and their families and carers. The team can support preparation for admission and Out Patient appointments, as well as supporting people whilst they are in hospital. The nurse liaises with clinic, ward teams, consultants and GPs, providing specialist support and advice such as helping with accessible information including pictorial care pathways, assessments, capacity and consent issues, communication and behaviour.

Home visits can be provided by this service.
Holds regular ‘development group’ meetings to inform external agencies and individuals of progress and plans.
