Parent Authorisation Form (PAF) Guidance for parents
Parent and carers need to complete a Parent authorisation Form (PAF) with a Suffolk childcare provider to be able to claim for Early Education Funding (EEF).
The PAF form should be supplied to you directly from your Suffolk childcare provider.
What is the purpose of the PAF form?
To authorise the childcare provider to claim early education funding from Suffolk County Council.
By signing the PAF, the parent/carer is agreeing that their chosen childcare provider can claim for the agreed number of funded hours.
The PAF confirms
- Eligibilty for early education funding
- The number of universal funded hours (eligible 2 year olds and 3 & 4 year olds are entitled to up to 15 Universal hours)
- The number of extended funded hours (working parents can apply for an additional 15 hours entitlement with HMRC, this is sometimes referred to as 30 hours)
- If parent/carers are claiming with one or more childcare providers.
- If a stretched funding offer has been agreed between the parent/carer and provider.
- The total number of universal entitlement and/or extended entitlement hours that the child will be receiving for the term.
Other information the PAF collects
- The PAF is used to collect the parent/carer information to enable an Early Years Pupil Premium (EYPP) check to be carried out.
- The PAF is used to collect information to see if the child is eligible for the Disability Access Fund (DAF)
- If eligible for DAF, the parent/carer can authorise their chosen childcare provider to claim the annual DAF funding.

What is a stretched offer?
A stretched offer means that parent/carers can use their funding by taking less hours each week but using it over more weeks.
A stretched offer agreement should be put in place to confirm how the hours would be used.
Parent/carers should ask their childcare provider about a stretched offer as not all childcare providers are able to offer this.
