Zest Young Adult Care - part of St Elizabeth Hospice

logo

St Elizabeth Hospice has been developing Zest since 2015, working closely with children’s hospice services to ensure young adults have a smooth transition into adult care.

Zest supports young adults aged 14 and upwards with progressive and incurable illnesses to ensure that they get the specialist care, tailored treatment and services they need to be able to enjoy life to the full.

Through this specialist care, young adults with challenging conditions are able to fulfil their aspirations, whether this is to manage their symptoms, access opportunities in the community and reduce loneliness by enabling them to meet and have fun with friends.

Zest is also about engaging with young adults through volunteering and fundraising through our shops and unique events - including fashion shows, auctions and attending festivals, such as Latitude.

Experience of including people with special educational needs and disabilities

We have been working with Zest young adult patients, their families and children’s services to develop our young adult service through co-production to ensure we are providing the right care and support for young adults and their families.

We offer a developmentally appropriate approach, tailored to each individual and their own aspirations for life.  We can offer regular reviews to support young adults and their families to achieve their goals and having the best quality of life.

We currently offer:

  • Short breaks - for 2 young adults, for 2 nights, 3 weekends a month
  • Zest Days - dedicated nurse-led specialist day service provision for those 18+, Fridays 10am-4pm
  • Zest X-Change - monthly evening social group for patients which may be the only social interaction they can safely access without parents or caregivers
  • Regular clinical reviews - access to the hospice multi-disciplinary team
  • A parent support group - a lifeline for families to share their experiences
  • Monthly Saturday whole family drop in session - as we know that everyone needs support
  • Transition coordination support - as they move from children’s hospice care to adult services
