Childrens Community Nursing Team (James Paget University Hospitals NHS Foundation Trust)

The children's community nursing services within Great Yarmouth and Waveney are easily accessible, and capable of meeting the needs of children, young people and their families in a timely way. The services respond to local needs, take account of the need to prevent hospital admission, facilitate early discharge and care for children with complex needs including those with life limiting conditions. The services are provided across a variety of settings including; hospital, home, respite and educational provision.

Children and young people who are disabled or who have complex health needs receive co-ordinated, high quality child and family centered services which are based on assessment of need, which promote social inclusion and where possible, enable them and their families to live ordinary lives. The community nursing service delivers this standard by assessing the nursing needs of sick children with and without disabilities and complex care needs and being part of the delivery of the co-ordinated Multi Agency Care. This includes individual packages of care delivered by carers trained by the community nursing team to meet individual care needs of a child or young person. This will also be delivered by participating in care planning with the local authority and training of their carers.

The children's community nursing service is staffed by appropriately qualified children's community nurses some of whom will have undertaken additional training and are able to demonstrate competency in a specialist field. Specialities include Diabetes, Epilepsy, Allergy, Asthma, Healthy Weight, Enuresis, Oncology, Encopresis, post surgical dressings and follow up.

Interpretation and translation services can be arranged by staff, for users of this service.

Home visits can be provided by this service in specific circumstances.
