Children's Community Nursing Team (CCNT) (James Paget University Hospitals NHS Foundation Trust)

The Children's Community Nursing Team (CCNT) service provides nursing care and support within the community setting, for your child aged 0 to 18 years (up to 19th birthday for children with learning disabilities) who live within Great Yarmouth and Waveney area.
The service responds to local needs, taking into account of the need to prevent hospital admission, facilitate early discharge and care for children with complex needs including those with life limiting conditions.
CCNT will meet the needs of your child and family by ensuring continuity of both educational and health services, providing the link between home, school, hospital and primary care and can provide an individualised healthcare plan for your child with chronic conditions under their care.
The team can provide advice, support, training to educational staff and medical professionals who care for your child, ensuring the care is safe at all times.
CCNT is staffed by appropriately qualified children's community nurses some of whom will have undertaken additional training in a specialist field. Specialities include Infant Complex care, Discharge planning, Diabetes, Epilepsy, Allergy, Asthma, Healthy Weight, Enuresis, Oncology, and Encopresis.
Your child will need to be referred to the service by any professional your child is under.

Interpretation and translation services can be arranged if English is not your first language.
Home visits can be provided to users of this service.
