Children's Community Medical Team

The Children's Community Medical Team is one of the services within Integrated Community Paediatric Services. The Community Medical Team carry out neurodevelopmental assessments of children and young people from birth until they leave school.  This is done to help identify a child or young person's needs, in order to ensure appropriate onward referral for treatment or further investigation. 

Children are referred if there is concern about their development.  This could be in the following areas:

  • speech and language
  • social communication
  • cognitive ability
  • hearing and vision
  • motor development

Common problems are developmental delay, cerebral palsy, speech or language disorders, learning difficulties, specific syndromes affecting development and Autism. 

The Children's Community Medical Team laso provides statutory services including Safeguarding, Adoption and Looked After Children Medicals and Statutory assessment for Special Educational Needs.

Children's Community Paediatric services are part of the local NHS health offer, and therefore no costs are incurred.

Referral and availability notes

Children under the age of 5 can be referred by their GP or HV, with parental consent.  School age children can also be referred by the school nurse, again with parental consent.  Referrals are also received from other health professionals including those within Integrated Community Paediatric Services.

All referrals are discussed at a weekly meeting, and accepted if they meet the criteria to access the service.  If not appropriate they are returned with advice and sign posting to alternative services.

Accreditation details

The Children's Community Medical Team consists of Consultant Community Paediatricians, Associate Specialist Doctors, Junior Doctors and Nurse Practitioners. All are registered health professionals.

Integrated Community Paediatric Services provides for the needs of children and young people with a range of additional needs, some of which are complex.    The Community Medical Team contribute to the Education Health and Care Plan assessment process as a statutory requirement.  All our staff are aware of the SEND reforms and are active participants where children/young people are known to their services.
