Community Children's Nursing Team

General Details

The Community Children’s Nursing Team (CCNT) care for children and young people in the community setting with clinical nursing needs. This includes those with short term nursing needs such as post-operative wound care, administration of intravenous medication and general nursing support and advice. In addition, the team will support children/young people who have very chronic/complex health care needs for example those who may be dependent on technology for their survival i.e. needing support with their breathing from a ventilator and those who have palliative care needs.

In addition, we have a team of specialist nursery nurses who specifically provide packages of care (mainly overnight) to children/young people who need very specialist support. These children have to meet a specified criteria and the care is designed to meet their specific need.

The team is a County wide service and covers the whole of Suffolk.


We have two bases, one located in Ipswich and the other in Bury St Edmunds with the Ipswich team covering the East of the County and the Bury team the West of the County, although both teams will work across the County.

The team will be as flexible as possible and will see families in a setting that is most suitable to them such as schools and in their own homes and allows families the choice regarding where they would like to receive their care.

Cost Details
Community Children's Nursing Team is offered within the local NHS health offer, and therefore no costs are incurred. Special funding arrangements are in place for particular packages of care offered by the team.
Referral required?
Referral and availability notes

To access the service, a referral can be made by any professional who can provide a summary of the child/young person's clinical nursing needs; this can be telephoned through to the team if urgent. A wide variety of nursing needs are catered for such as gastrostomy care, wound care, administration of medications and many other nursing procedures that can safely be carried out outside of the hospital environment. Each referral is considered by a qualified children’s nurse who will be able to determine whether access into the service is appropriate. Once the referral has been received contact will be made with the family. If the referral is urgent then contact will be made on the same working day but all families will be contacted within 48 hours. Initial contact is most likely to be by telephone to arrange an appointment for the first visit. During the first visit a full nursing assessment will be carried out and a care plan will be developed in full consultation with the family and child/young person. If necessary an interpreter can be used and it is possible to have care plans written in different languages.

All nursing members of the team are fully qualified paediatric nurses and are competent in a range of clinical skills. In addition, the team has qualified Specialist Nursery Nurses who are specially trained to care for children/young people with complex health care needs in the community setting. The Specialist Nursery Nurses work independently and carry out specific nursing procedures as well as delivering respite care to children/young people who meet a specified criteria. All staff within the team have access to regular training and supervision to ensure that their skills are continually updated and that they are competent in all aspects of care.

Experience of including people with special educational needs and disabilities

Integrated Community Paediatric Services provide for the specialist health needs of children/young people across Suffolk (excluding Waveney). All staff are aware and actively engaged in the SEND reforms.  Where appropriate staff will contribute to the assessments for Education Health and Care plans.
