Acute Liaison Nurse for Adults with Learning Disabilities (James Paget University Hospitals NHS Foundation Trust)

General Details
The Liaison nurse works with people who have learning disabilities and/or autism, their families and carers in their preparation for admission or at Out Patient appointments.
The nurse liaises with clinic, ward teams, consultants and GPs, facilitating pre-operation visits and helping with accessible information including pictorial care pathways, assessments, capacity and consent issues.
Home visits are provided by this service.
Interpretation and translation services can be arranged by staff, for users of this service.
