Looked after Children Health Team (East Coast Community Healthcare CIC)

It is important to note that ECCH Looked After Children service is a referral in service. Children are referred in from the Local Authority, Children's Service. Once the child/young person is referred in then the service can accept referrals from other professionals i.e. GP's/Social Workers. The referrals will be specific regarding the identified child.

The Looked after Children service ensures that all children who enter the care system have an Initial Health Assessment which is completed by a registered medical Practitioner; this will be completed at the James Paget Hospital and within a clinical setting.

Following the Initial Health Assessment with the James Paget Hospital (with admin support and coordination from East Coast Community Health), if the child/young person remains within the care system, each subsequent review health assessment will be completed either by an ECCH Health Visitor every six months (if the child is under 5 years of age) or completed by a ECCH Specialist LAC Nurse annually (for children between the ages of 5-18)

The nurses will also support and offer training to Foster carers and other professionals.

The ECCH Looked after Children service consists of three qualified nurses and two administrators. The service works very closely within the multi-agency forum to ensure timely completion of the health assessments and appropriate referrals as required.

Home visits are provided by this service.
