Professionals

Any health or social care professional can make a professional referral. The most important criteria is that you know the person to best understand their needs and are supporting them to access a service they might find helpful. You may work in a college, medical practice, community health practice, acute hospital, or housing provider etc. All you need to do is fill out the professional referral form here for the person you have in mind.

You can find useful resources on the Wellbeing website to help support you on identifying how and when to make a referral. Access those resources here.
