The service aims to reduce the impact of disability and ill health through comprehensive assessment, rehabilitation intervention and a case management approach. The focus is on supporting people to remain living at home, to life’s end in some cases. The service plays a key role reducing avoidable and inappropriate acute admissions.
The objectives of Occupational Therapy Service are to provide assessment, treatment and advice following change in health status including:
* Personal Care and functional mobility to enhance occupational performance
* End of life care with health needs
* Admission avoidance
* Enabling independence
* Lifestyle management, Self-Management and Health Promotion
* Support and education for informal carers
Interpretation and translation services can be arranged by staff, for users of this service.
